　　4) Name, address, production address, contact information, production license number or production record certificate number of the manufacturing enterprise;

　　8) Necessary warnings and precautions;

　　11) The label of medical devices with radiation should be accompanied by Chinese warning signs or instructions. If the label of the medical device cannot display all the above content due to location or size limitations, it should at least indicate the product name, model, specifications, production date, service life or expiration date, and clearly indicate "Other content can be found in the manual" on the label.
